shell is unfired hanging out ejection port and bolt closed on it. happens on third shot using 3.5 blackcloud

I posted this in the waterfowl section also but have a new sbe ii with around 3 boxes of shells fired has already jammed 4x.. the third shell is unfired and jammed hanging out.... any suggestions?

I have a new sbe II that has around 3 boxes of shells through it and have had around 4 jams where on the third shell unfired is hanging out and jammed. any suggestions?
