SBE11 want chamber?

I have a SBEll I bought less than 6mnths ago. My problem is I load 2 in the mag. then I chamber 1 then I load my 3rd shell. When by chance I get to shoot my 3 shells the 1st 2 fire and the 3rd is still in the mag. Does anyone know what my problem is (spring)? I'm using 3 1/2" Black Cloud.
