It includes the 4 shot extension tube, end cap, spring and barrel mounting bracket. These were being sold years back by a local dealer in my area located in Carpentersville, IL. This item can be picked up locally (my zip 60004) or I will be happy to ship it, the buyer is responsible for shipping!
