Benelli Super Nova shell fall out of loading hatch

On my Super Nova after inserting the shells and then trying to load one in the chamber, the shell will fall out of the bottom. Any ideas.

It is hard understand the issue; are you loading the magazine and then putting one in the chamber?  I usually do the opposite- load one in chamber, close bolt ( safety on), then load the magazine tube.

Your Carrier is out of adjustment, which is not allowing the part to return to typical battery position.
